Having been marketed to millions as a weight-loss solution, the Atkins diet is now presented as a means of preventing type 2 diabetes, a disease whose rates are skyrocketing, thanks to the prevalence of risk factors such as obesity and high lipid and blood sugar levels. The authors clearly outline their interpretation of the path to diabetes, arguing that carbs cause blood sugar spikes, triggering the release of more insulin—the hormone that regulates blood sugar—than the cells need. Blood sugar ends up stored as fat and the body's cells start responding more slowly to the insulin, which leads to elevated blood sugar levels. But the tone the authors adopt when touting their low-carb, high-protein, high-fat approach may arouse some skepticism and even fear. "In the end, only you can decide what's best for your health," they warn. "You can choose the Atkins approach and improve your health, or you can choose the ADA [American Diabetes Association] approach and descend into more and more medications and poor health." While studies now demonstrate low-carb dieting can lead to weight loss and cholesterol control over the short term, experts tend to agree that diets that demonize one food group aren't easy to stick to over the long haul. Still, the work includes valuable nutritional information and sounds a needed alarm about the diabetes epidemic.

If smoked salmon and cream cheese omelets, sautéed jumbo shrimp, and double-patty burgers suit your palate, belly up to the Protein Power diet: "Not a high protein diet" but "an adequate protein diet." Similar to Dr. Robert Atkins's New Diet Revolution, the authors cite insulin as the main culprit in weight gain and expound the benefits of a diet extremely low in carbohydrates. Carbohydrates, which are changed into sugar during digestion, stimulate the body to store fat, making weight loss virtually impossible. The most revolutionary idea put forth in Protein Power is that the fat you eat has very little bearing on the fat you gain: in other words, we aren't what we eat after all.    According to author Loren Cordain, modern health and diet problems didn't start with the advent of packaged snack food, but much earlier--back at the dawn of the agricultural age many thousands of years ago. As humans became less nomadic and more dependent on high-carbohydrate diets, we left behind the diet we had evolved with, which is based on low-fat proteins and plenty of fruits and vegetables. Sugars, fats, and carbs were rare, if they were present at all, and survival required a steady, if low-key, level of activity. Cordain's book The Paleo Diet blends medical research with a healthy sprinkle of individual anecdotes, practical tips, and recipes designed to make his suggestions into a sustainable lifestyle, rather than a simple month-long diet; he even includes cooking recommendations and nationwide sources for wild game. Claims of improving diseases from diabetes to acne to polycystic ovary disease may be a little overstated, but in general the advice seems sound. Can any of us really go wrong by adding lots more vegetables and fruits to our daily regimen? One recommendation on safe tanning with a gradual reduction in sunscreen is surprising and not much detail is provided for safety issues that can accompany increased sun exposure. Dr. Mary G. Enig, a nutritionist/biochemist of international renown for her research on the nutritional aspects of fats and oils, is a consultant, clinician, and the Director of the Nutritional Sciences Division of Enig Associates, Inc., Silver Spring, Maryland. Dr. Enig, a consultant on nutrition to individuals, industry, and state and federal governments, is a licensed practitioner in Maryland and the District of Columbia. She has served as a Contributing Editor of the scientific journal Clinical Nutrition and a Consulting Editor of the Journal of the American College of Nutrition. Dr. Enig has authored numerous journal publications, mainly on fats and oils research and nutrient/drug interactions, and is a well-known invited lecturer at scientific meetings and a popular interviewee on TV and radio shows about nutrition. She was an early and articulate critic of the use of trans fatty acids and advocated their inclusion in nutritional labeling; the scientific mainstream is now challenging the food product industry's use of trans-containing partially hydrogenated vegetable oils. Starred Review. Taubes's eye-opening challenge to widely accepted ideas on nutrition and weight loss is as provocative as was his 2001 NewYork Times Magazine article, What if It's All a Big Fat Lie? Taubes (Bad Science), a writer for Science magazine, begins by showing how public health data has been misinterpreted to mark dietary fat and cholesterol as the primary causes of coronary heart disease. Deeper examination, he says, shows that heart disease and other diseases of civilization appear to result from increased consumption of refined carbohydrates: sugar, white flour and white rice. When researcher John Yudkin announced these results in the 1950s, however, he was drowned out by the conventional wisdom. Taubes cites clinical evidence showing that elevated triglyceride levels, rather than high total cholesterol, are associated with increased risk of heart disease-but measuring triglycerides is more difficult than measuring cholesterol. Taubes says that the current U.S. obesity epidemic actually consists of a very small increase in the average body mass index. Taube's arguments are lucid and well supported by lengthy notes and bibliography. His call for dietary advice that is based on rigorous science, not century-old preconceptions about the penalties of gluttony and sloth is bound to be echoed loudly by many readers. Illus. (Oct. 2)
